Admission and Enrollment

The following forms need to be completed prior to attending the center.

	1. Handbook read and parent agreement signed.
	2. Enrollment papers filled out and signed.
	3. Required immunizations specified by the Florida Department of Health, TB test if required by the Florida Department of Health. Please remember to update all shots as soon as possible so we can keep your child's records current.
	4. Document stating that the child is able to take part in the program from licensed health personnel.
	5. Signed consent for child to receive emergency medical treatment, medicine, and permission for transportation.

It is the parent's responsibility to inform the center of any change such as new address, new phone numbers (home, work, and cell), or any other information that would be needed in order to contact the parent or release the child to the appropriate person. The center will notify the parents in writing of any policy changes made. The parents will need to sign off upon receiving and reading any policy changes.
